Print out and complete 42 NPRS and 13-A5.1. (Requires Adobe Acrobat Reader)
Obtain a suitable photo of yourself.
(This must be an actual photo, not a drawing or other representation, and must be of the received citizen.)
Obtain five (5) United States of America dollars.
(If you live outside the United States of America, the rough equivalent of your own home currency is perfectly acceptable.)
Postal mail the completed forms, your photo, and the currency to:
